Notification form for financial instruments transactions in one's own issuing institution

(Section 5:60 of the Financial Supervision Act (Wft).

ATTENTION: The notification may be delayed until a time:
at which the transactions performed for one's private account by individuals defined under 1,2 or 3 (part II, notification form), in the applicable calendar year reaches an amount of 5,000 euro or greater per individual; or at which the transactions performed for one's private account by individuals defined under 1,2 or 3 (part II, notification form), adding up the private transactions and the transactions by related persons within the meaning of 4 to 7 (part II notification form), in the applicable calendar year reaches an amount of 5,000 euro or greater.
