Pursuant to Section 5a Wte 1995, securities issuing institutions are required to file their financial reporting with the AFM within eight days of the adoption of the financial statements. The financial reporting consists of the adopted financial statements, director’s report and other information required in Section 392 of Book 2 of the Netherlands Civil Code. In addition, Section 3 of the Act on the Supervision of Financial Reporting requires companies to file with the AFM any statement made public by an issuer following a recommendation by the AFM.

For filing hard copy financial reports pursuant to, please enter the required information in the text entry fields. All entry fields are required. Send the completed and signed form and the hard copy financial reports to: Netherlands Authority for the Financial Markets, Department Melden en Registreren, P.O. Box 11723, 1001 GS  AMSTERDAM. The AFM will process the filed hard copy documents to the public registers available on the AFM-website.
